(Department request for change of Faculty Advisor when a student wishes to change his/her academic advisor within the deparmtent).

CLEP Exam for Credit

Through College-Level Examination Program (CLEP), students may earn college credit for knowledge acquired outside the classroom.

(Withdraw from course (class); Grade of W will be reflected on academic transcript)

Declaration (Change) of

Major / Minor / Concentration

(Degree seeking students must declare a major field of study no later than the end of sophomore year or upon completion of 60 credit hours; Declaration/Change of major forms are processed by the Office of the Registrar)
